Responsibilities

  • Provides referrals and resources based on family needs.
  • Aids in monthly billing documentation and data collection.
  • Performs a variety of clerical duties, including collection and data entry for the home visiting program.
  • Follows up with referrals and identified issues.

Community Outreach

  • Serves as a liaison for the community by building, working with, and establishing relationships.
  • Affords assistance in the development of community support and education groups.

Operations

  • Demonstrates focus on strengthening parent-child relationships (improving parenting skills, reducing family stress, and meeting the bio-psychosocial needs of infants, children, and their parents).
  • Coordinates play groups.
  • Displays advanced training and skills in development, children with special needs, family retention, advanced substance abuse, and family violence.
  • Performs initial assessment and evaluation to potential, eligible families.
  • Collaborates with Program Manager/Supervisor to discuss and review each referral.
  • Affords assistance in client satisfaction phone calls and completes appropriate documents.
  • Affords assistance in completing home observations for staff and completion of documents.
  • Ensures appropriate curriculum is based on individual family needs.
  • Maintains positive community relationships.
  • Develops and maintains family plan of care progression and/or accomplishments by families.

Staff Development

  • Affords assistance to the Supervisor in staff development.
  • Affords assistance to the Supervisor in participation in orientation of new staff.
  • Affords assistance to the Supervisor in scheduling education opportunities for staff.

Compliance/Safety

  • Reports any safety-related incidents in a timely fashion through the Midas/RDE tool.
  • Attends all safety-related training programs.
  • Performs work in a safe manner.
  • Monitors work environment for possible safety issues and ensures others are also performing work in a safe manner.
  • Stays current and complies with state and federal regulations/statutes and company policies that impact the employee's area of responsibility.
  • If required for the position, ensures all certifications and/or licenses are up-to-date and valid prior to expiration dates.
  • Completes all company mandatory modules and required job-specific training in the specified time frame.
