About the role
Gilead Community Services is seeking a caring and recovery-oriented Senior Clinical Case Manager to join our multi-disciplinary supported apartment program staff. The position is located in the Middletown area and works Monday through Friday from 7a-3p (40 hours).
Responsibilities
- Provide clinical knowledge regarding severe and prolonged mental health and substance abuse disorders through training and education.
- Support the Director/Manager in leading an effective program, including supervising shift activities, reviewing, and implementing the daily schedule, and providing clinical leadership.
- Work with the treatment team to manage clients' mental health and assess for risks, implementing risk management plans as necessary.
- Provide recovery-oriented clinical case-management services with an emphasis on dignity, respect, and inclusion of family and other natural supports.
- Provide support, skills training and education in areas including daily living skills, nutrition, financial management and symptom management.
- Monitor positive outcome measures and ensure program compliance through accurate and timely data reporting.
- Complete assessments, treatment plans, and other documentation electronically and on paper in a timely manner.
- Act with compassion, integrity, and ethics.
Requirements
- Bachelor’s degree in a human service field; Master’s degree preferred.
- Experience working with individuals with co-occurring disorders.
- Three years of clinical experience.
- Maintain and provide proof of valid driver’s license and automobile insurance in good standing.
- Reliable use of personal vehicle as needed.

Pay
$21.17 per hour.
