About the role
Under the general supervision of the Director of Clinical Services, the Scheduler/Intake Coordinator coordinates clients’ transition from an institutional setting to home health care services, processes client referrals, and initiates services according to prescribed policies and procedures, including all applicable state, federal, and accreditation regulations.
Responsibilities
- Collects database information during the intake process.
- Assists the client, physician, and/or discharging facility to establish an effective home care/hospice plan prior to the client’s transfer to the home setting.
- Communicates with and educates referral sources regarding services via written and telephone contact.
- Schedules staff for Medicare, Private Duty, and Passport.
- Serves as a liaison between referral sources and interdisciplinary teams.
- Investigates, verifies, negotiates, and documents coverage from all payor sources.
- Advocates on behalf of clients to ensure receipt of entitled benefits.
- Tracks all referrals/admissions on the weekly referral report.
- Updates referral sources on the status of clients currently receiving service or recently discharged and the client’s response to services received.
- Communicates appropriate information to team members regarding new admissions and referrals, including client needs such as a plan of care, supplies, and equipment.
- Provides appropriate documentation of referrals per agency policies.
- Participates in agency quality assurance and performance improvement programs.
- Communicates necessary information to other departments.
- Assists in developing and modifying policies and procedures.
- Attends and participates in regularly scheduled interdisciplinary team meetings to coordinate care plans, follow up on changes, and receive staff support and in-service education.
- Demonstrates and maintains a positive, professional manner; manages time efficiently; provides leadership as a role model to promote a team approach; collaborates in policy creation and problem-solving.
- Adheres to all policies, procedures, terms, and conditions set forth in the Employee Information Guide, Code of Conduct, and facility handbook, including corporate compliance, drug-free workplace, safe work practices, and all federal, state, and local regulations and laws.
- Ensures all care is provided with respect and dignity for residents/clients, their families, and staff; reports complaints, allegations of abuse, misappropriation of funds/property, and other compliance concerns.
- Completes all annual education requirements, including Compliance, Privacy, Security, and HIPAA training, and exhibits behavior as set forth by the Code of Conduct.
- Performs other duties as assigned.
Requirements
- High school diploma or GED.
- Prior scheduling experience in the health care field preferred.
- Minimal travel up to 10%.
- Good communication, comprehension, computer, and interpersonal skills.
- Ability to speak, read, write, and understand English.
- Working knowledge of Windows, Microsoft Office Suite, and the World Wide Web.
