Full-Time RN Case Manager

Responsibilities

• Coordinate care for adult rehab patients, including assessments, care planning, and safe discharges.
• Collaborate with physicians, therapists, social workers, and families to optimize recovery and support patient goals.
• Perform utilization review, documentation, and patient and family education.
• Coordinate community resources to enhance patient care.

Qualifications and Skills

• Registered Nurse (RN) license.
• Experience in patient care coordination, rehabilitation, or case management preferred.
• Strong communication and collaboration skills.
• Knowledge of documentation, utilization review, and patient education.
• Ability to work in a team‑oriented, patient‑first environment that values integrity and evidence‑based practice.
