Full-Time RN Case Manager

About the Role

We are seeking a skilled RN Case Manager to support both inpatient and outpatient care management, including utilization management and discharge planning. The role involves collaborating with multidisciplinary teams, supporting patient transitions, and ensuring safe discharges across all age groups.

Responsibilities

• Conduct daily rounds in inpatient settings to coordinate care and facilitate safe discharges.
• Collaborate with multidisciplinary teams to support patient transitions.
• Perform utilization management and discharge planning for inpatient and outpatient care.
• Inpatient duties: primarily in-person work, with some weekend and holiday on-call requirements.
• Outpatient duties: focus on care plan facilitation, referrals, and remote work with no weekend or on-call requirements.

Requirements

• Nursing degree (Associate or Bachelor’s).
• Active RN license.
• At least one year of related experience in case management, utilization management, or discharge planning.

Qualifications (Preferred)

• Bachelor’s degree in Nursing.
• Bilingual skills.
• Relevant certifications (e.g., CCM, ACM).

Skills

• Strong communication and care coordination abilities.
• Proficiency with electronic medical records (EMR) and software systems.
