Full-Time RN Case Manager

Responsibilities

• Provide compassionate home visits to deliver skilled nursing care
• Conduct comprehensive patient assessments
• Develop and modify individualized care plans
• Collaborate with physicians, therapists, and family caregivers
• Educate patients on disease management
• Ensure accurate, regulatory-compliant documentation
• Support a patient-centered culture emphasizing respect and clinical excellence

Qualifications & Skills

• Registered Nurse (RN) licensure
• Strong assessment, communication, and patient education skills
• Ability to work independently in a field-based setting
• Knowledge of regulatory standards and documentation requirements
