Full-Time RN Case Manager

Role: RN Case Manager responsible for providing comprehensive primary care to 25-35 home-based patients, coordinating multidisciplinary services, supervising LPNs and Home Health Aides, and ensuring patient goals are met. Responsibilities Assess, develop, and manage patient care plans following physician orders using the nursing process.Coordinate services across disciplines, revise care plans as needed, and monitor patient progress from admission to discharge.Supervise and support LPNs and Home Health Aides, ensuring quality care delivery.Provide patient and caregiver education to promote self-care and independence.Communicate patient status and changes with the care team and physicians.Ensure compliance with regulatory standards, complete documentation accurately, and participate in performance improvement activities.Support staff training, participate in meetings, and perform other duties as assigned. Qualifications & Skills Graduate of an approved nursing program; valid RN license (single state or multi-state compact).Minimum 1 year of Medical/Surgical nursing experience; home health and ICU experience preferred.Strong critical care assessment, clinical, leadership, organizational, and communication skills.Proficient in using computers and home health software; knowledge of OASIS and regulatory guidelines is a plus.
