Evaluate each client’s condition and create personalized care strategies to promote overall well-being
Collaborate with healthcare providers and support staff to deliver and oversee care services
Continuously revise care approaches based on changes in client status and response to treatment
Guide clients and families on medical conditions, medications, and effective care routines
Document all interactions thoroughly while adhering to clinical standards and home care requirements
Active Registered Nurse (RN) license in the state of MN required
Home care experience required
Must be 21 years of age
Competitive Pay: $87,880 - $93,662/year + credit for experience
Schedule: This is a full time position, Monday - Friday | On call rotation required
