About the role

Stamford Health is hiring a full-time Nurse Case Manager to join our Case Management Team!

Responsibilities

  • Carefully coordinate the care delivered to an assigned group of patients.
  • Collaborate with the health care team to assess, plan, and evaluate patient outcomes.
  • Guide and direct the care provided by other nurses and health care team members to ensure quality and cost-effective care.
  • Develop and implement the care plan within the care continuum, incorporating patient and family input.
  • Maintain ongoing communication with physicians, referral sources, caregivers, patients, and families to facilitate coordination and transitional care.
  • Communicate effectively and clearly, independently initiate assessments, identify alternatives, and choose appropriate courses of action.
  • Evaluate the effectiveness of interventions and act as an advocate for an individual's health care needs.
  • Identify and address barriers to patient access to care and actively participate in the revenue process, including the appeal/denial process.
  • Act as a resource for members of the healthcare team and participate in local professional organizations and activities.

Qualifications

  • Graduate from an accredited School of Nursing.
  • Minimum of 2 years acute care HOSPITAL experience required.
  • Valid license as Registered Nurse in Connecticut.
  • Bachelor's prepared Registered Nurse licensed in the state of Connecticut is required.
  • Strong clinical experience in an acute care setting with additional experience in home care, long-term care, and case management is preferred.
  • Professional certification in case management or clinical specialty is highly desirable.
  • Prior case management, utilization management, or discharge planning experience preferred.
  • Experience with multi-aged and multicultural populations.
