Full-Time RN Case Manager

About the role

We are seeking a Registered Nurse Case Manager to provide comprehensive hospice care, including developing and implementing personalized plans of care that promote physical, psychosocial, emotional, and spiritual well-being.

Responsibilities

• Conducting initial assessments within 48 hours of referral
• Ongoing re-assessment every 15 days
• Coordinating care with patients, families, physicians, and interdisciplinary team members
• Documenting patient progress
• Educating families on self-care techniques
• Adapting to diverse home environments and cultural sensitivities

Qualifications

• Current RN licensure in the practice state
• Valid driver's license
• At least 2 years of RN experience

Skills

• Clinical hospice experience (preferred)
• CPR certification (preferred)

Pay

$75,624 to $87,930 annually
