Full Time - Registered Nurse (RN)

About the Role

Provide home healthcare services as a Registered Nurse, delivering care in accordance with physician orders, agency protocols, and regulatory standards.

Responsibilities

• Administer physician orders following agency and regulatory standards.
• Develop and update patient care plans at least every 60 days.
• Document patient care comprehensively from admission to discharge using Oasis assessment tools.
• Serve as a case manager, coordinating multidisciplinary care.
• Participate in quality assurance programs.
• Maintain professional documentation within policy guidelines.

Requirements

• Active New York State RN license.
• Valid New York State driver’s license.
• CPR/BLS certification.
• COVID vaccination.
• At least one year of RN experience preferred (new graduates and those with homecare, acute, or rehab experience are encouraged to apply).
• Oasis knowledge is advantageous.
