Full Time - Registered Nurse (RN)

About the Role

Provide in-home patient care within regulatory guidelines as part of a supportive team environment. This in-person role focuses on delivering high-quality care and supporting career growth in the home healthcare setting.

Responsibilities

• Administer physician orders and develop and review care plans every 60 days.
• Document patient care comprehensively using Oasis assessment tools.
• Serve as case manager, coordinating multidisciplinary care.
• Participate in quality assurance initiatives.
• Ensure professional documentation within policy timelines.

Requirements

• Active NYS Registered Nurse license.
• Valid NYS Driver’s License, CPR/BLS certification, and COVID vaccination.
• Preferably one year of RN experience; new graduates are welcome.
• Previous homecare, acute care, or rehab experience is advantageous.
• Oasis knowledge is a plus.
