About the Role
Provide in-home patient care as a Registered Nurse, implementing physician orders and adhering to regulatory standards within your scope of practice.
Responsibilities
- Implement physician orders and adhere to regulatory standards within scope of practice.
- Develop and review care plans at least every 60 days.
- Document patient care comprehensively from admission to discharge using Oasis assessment tools.
- Serve as a case manager, coordinating multidisciplinary care plans.
- Participate in quality assurance programs and maintain professional documentation.
Requirements
- Active NYS Registered Nurse licensure and registration.
- Valid NYS Driver’s License, CPR/BLS certification, and COVID vaccination.
- One year of RN experience preferred; new graduates are welcome.
- Previous homecare, acute, or rehab experience is a plus.
- Knowledge of Oasis assessments is advantageous.
