Summary of Physical Therapist Job Description
The Physical Therapist will provide outpatient physical therapy services for Odom Rehab in an ethical and professional manner, adhering to state guidelines. They will evaluate and treat patients, establish appropriate Plans of Care, and reassess periodically to ensure goals remain relevant. They will oversee PT assistants, deliver treatment, educate patients and families, update plans as needed, assess adaptive equipment, develop discharge plans, document daily, submit charges, communicate with physicians, and assist in protocol development. They must maintain an active Physical Therapy license and continue education.
Essential Functions
- Evaluate and treat patients in accordance with state guidelines, identifying root causes of dysfunction and translating them into impairments in daily function.
- Establish and update Plans of Care for patients, outlining the path to achieve their goals.
- Deliver treatment including manual therapy, therapeutic exercises, and training on proper techniques and body mechanics.
- Address pain and educate patients on self-management strategies, as well as educate family members.
- Assess and recommend appropriate adaptive equipment, such as canes, walkers, orthotics, etc.
- Develop a discharge plan for patients once they complete their physical therapy.
- Document patients' treatment daily based on established guidelines.
- Communicate regularly with physicians and medical staff regarding patients' status or changes in status.
- Aid in developing and updating Odom Rehab treatment protocols as needed.
- Participate in staff meetings and track Continuing Education Credits.
- Maintain an active Physical Therapy license.

Qualification Requirements
- Certifications: Graduation from an accredited Physical Therapy program with a degree in Physical Therapy and current MN State Physical Therapy Licensure.
- One (1) year of experience as a Physical Therapist (preferred).
- Recent / previous work experience in primary health care setting (preferred).

Working Environment and Physical Requirements
Work Rating: Medium
Medium work involves lifting no more than 50 pounds at a time with frequent lifting or carrying of objects weighing up to 25 pounds. If someone can do medium work, they can also do sedentary and light work.
Driving Requirements
This position has been designated as Mandatory Driving: Administrative. As part of the new hire onboarding process, OHW conducts background checks including a review of driving records for all employees required to drive in their position. To be in good standing, you must maintain a satisfactory driving record. OHW will assess individual driving records to determine if a person meets OHW’s driver safety requirements. If an individual’s driving record indicates a violation or combination of violations that are deemed excessive, OHW may discipline the employee up to and including termination of employment.
