Seeking an experienced Registered Nurse for a leadership role in home care services. Responsibilities Include Coordinating, implementing, and evaluating patient care plans at homeConducting patient assessments, managing medications, and providing education to patients and familiesReviewing insurance information and ensuring effective communication among caregiversSupervising and mentoring new nurses and peers during field visits Qualifications Graduate from an accredited School of Nursing; BSN preferredMinimum of 3 years of home care experience; supervisory experience preferredValid NYS RN licenseDriver's license and access to a vehicleKnowledge of Home Health regulations and Oasis Certification preferredStrong communication, documentation, and time management skillsProficiency in EMR and Word software This role requires compassion, clinical insight, strong decision-making, and leadership abilities to ensure quality patient care in a home setting.
