Full-Time LPN 4:45pm-5:15am - Skilled Nursing

Homestead Village is seeking an experienced Licensed Practical Nurse to work in one of our skilled nursing households. Nurses with exceptional skills, a passion for serving older adults, and excitement for leading a small team of caregivers would be a great fit for this position.

Schedule

Full-time, non-exempt; 4:45pm–5:15am — three 12-hour shifts per week including every third weekend and every other holiday.

Location

The Apostle Center for Care, 56-bed Skilled Nursing Facility.

Pay

Base pay determined by years licensed; newly licensed nurses with less than one year of experience start at $28.00/hour. Additional differentials apply:

• $1.50/hour from 3:00pm–11:00pm
• $0.75/hour from 11:00pm–7:00am
• $1.25/hour on weekends (Saturday & Sunday)

Qualifications

• Currently licensed in the Commonwealth of PA as a Licensed Practical Nurse
• Minimum of three years as an LPN preferred
• Minimum of three years of supervisory experience preferred
• Previous experience in a skilled nursing facility preferred
• Familiarity with SNF Regulations
• Ability to work cooperatively with team members and compassionately with residents
• Ability to actively direct subordinates in a professional and supportive manner
• Able to perform the essential job functions of the position

Responsibilities

• Establish respectful, warm relationships with residents; perform rounds
• Perform all aspects of resident care in accordance with nursing practices and policies
• Complete all required documentation
• Communicate readily with team members and RN Nurse Leaders
• Administer medications and perform treatments according to policy, physician’s orders, and federal & state guidelines
• Observe and report changes in residents’ condition immediately
• Assist in training, supervising, and evaluating assigned staff, including participation in orientation of new employees
• Assist with emergency response campus-wide, as needed
• Attend all required staff meetings and complete monthly education on Relias by due dates
• Maintain a positive and professional manner at all times
• Perform other duties as assigned
