What You'll Do Provide skilled nursing visits in patients' homesDeliver nursing care according to physician orders and the established plan of careMonitor patients' conditions and report significant changes to the RN Case Manager, physician, and appropriate members of the care teamAdminister medications and treatments as orderedPerform wound care and other skilled nursing procedures within the LPN scope of practiceEducate patients and caregivers on medications, disease management, safety, and home care techniquesReinforce patient care plans and goalsCommunicate and collaborate with RNs, physicians, therapists, and other members of the interdisciplinary teamComplete accurate and timely clinical documentationHelp patients safely maintain their health, independence, and quality of life at home What We're Looking For Current Arizona LPN license in good standingHome Health experience preferredStrong clinical assessment and communication skillsExcellent organizational and time-management abilitiesAbility to work independently in a field-based environmentValid driver's licenseReliable transportation and current auto insuranceComfortable traveling throughout the Sahuarita and Green Valley service areaCompassionate, dependable, and committed to providing quality patient care Why Join Dependable Health Services?
