Full-Time Home Health Intake Coordinator

Responsibilities

• Receive and process new referrals in a timely manner.
• Facilitate care coordination with sales reps, referral sources, physician offices, patients, and agency clinical leaders.
• Effectively communicate and relay accurate care coordination details to internal and external constituents in the referral process.
• Take ownership of recording accurate information on the referral tracker and in the EHR, ensuring all referrals move through the process quickly.
• Respond and communicate in a professional manner.
• Demonstrate commitment, competency, and ability to handle a high volume of details proficiently.

Requirements

• High school diploma or equivalent.
• 2–3 years of home health intake experience preferred; LVN candidates accepted.
• Required computer proficiency and excellent interpersonal, organizational, and phone skills.
• Ability to work at a fast pace.
