About the role

Join our team as a Heath and Wellness Coordinator to make a difference in the lives of seniors in a senior living building.

Responsibilities

• Affirms implementation of policies and procedures relating to Resident care.
• Supervises and assists in provision of all Resident services, including coordination with independent health care providers.
• Conducts assessments, negotiates service plans, and updates service information based on Resident needs.
• Affirms timely assessments of Residents and ensures appropriate charging based on assessments.
• Affirms Social History Assessments and outlines life enrichment programming, meeting with residents and families, and implementing and following up with associates and volunteers on life enrichment programming.

Requirements

• Must be an LVN or LPN.

Qualifications

• Strong interpersonal skills.
• Experience in healthcare settings preferred.

Pay

Starting Wage: $65k

Schedule

Shift: Monday through Friday 8:30am to 5:00pm
