About the role
The Health Services Coordinator plays a crucial role on the health services team, supporting the department's operations through various administrative and clinical functions.
Responsibilities
- Conduct and coordinate assessments / evaluations of potential residents and make recommendations for admission.
- Draft initial individualized Service Plans and update as needed.
- Review service plans with responsible parties and Executive Director as requested by DHS.
- Ensure continuity of the assisted living residents’ total care regimen.
- Provide training and education on essential topics including medication delivery, treatments, safety protocols, emergency procedures, and state requirements.
- Perform all assigned duties accurately and timely, including required documentation.
- Aid in the community marketing effort through positive interactions and serving as a liaison.
- Maintain a safe and secure environment following established safety standards, policies, and procedures.
- Understand and comply with all Federal, State, and local regulations, and company policies and procedures.
- Support a spirit of teamwork and open communication in accordance with MBK principles and core values.
Requirements
- RN, LVN, or LPN License that is active and in good standing is essential; adherence to all requirements to maintain license including CEU completion and timely renewal.
- 2+ years of prior related work experience in a similar healthcare environment.
- Must complete required Background clearances, health screening, and provide negative TB test results within 7 days of employment (must be within the last 6 months).
- Valid state driver’s license and reliable method of transportation required.
- Competent and current technical and computer skills, including familiarity with Microsoft Office Suite (e.g., Word, Excel, Outlook, etc.) and office equipment.
- Excellent communication skills including the ability to speak, write, and read English.
- Ability to make sound, independent decisions when circumstances warrant, and remain calm during stressful or emergency situations.
- Ability to deal tactfully and professionally at all times with personnel, residents, family members, and guests.
- Strong organizational, problem-solving, and time management skills.
- Maintain a neat and organized work environment to promote safe coordination of resident care.
Physical Demands & Work Environment
- Must be able to work a flexible schedule, including weekends and holidays.
- Mobile and able to perform the physical requirements of the job, including walking, bending, kneeling, squatting, pulling, reaching overhead, and repetitive motion.
- Ability to move intermittently throughout the workday, in the community and between neighborhoods.
- Ability to lift and carry up to 50 pounds and push up to 250 pounds.
- Ability to assist in the physical movement of residents during routine transfers or in emergency situations.
Education Requirements
