Full-Time Emergency Room Care Manager

Role: RN Care Manager in the Emergency Room Responsibilities Coordinate care for high-acuity, diverse patients in a fast-paced, safety-net environment.Perform comprehensive assessments and develop patient-centered care plans.Support safe transitions of care and collaborate with physicians, social workers, and community partners.Educate patients and families, advocate for resources, and utilize evidence-based practices.Contribute to improving patient outcomes, reducing avoidable readmissions, and promoting equitable, compassionate care.
