Full-Time Case Manager

About the role

This role requires critical thinking, effective communication, and decisive judgment to build positive relationships and lead others. The incumbent will provide education and resources on care progression, utilization of services, appropriate levels of care, and safe patient transitions to patients, families, and healthcare teams.

Qualifications

• Minimum one year experience in hospital-based Case Management or five years in healthcare
• Graduate of an accredited Registered/Professional Nursing Program
• Basic Life Support (BLS) Healthcare Provider certification required upon hire
