Full-Time Case Manager

Responsibilities

• Assist clients in identifying services, programs, and resources to promote independence and achieve personal goals.
• Provide support, advocacy, referral, and assessment services tailored to individual needs.
• Understand clients' diagnoses and recommend appropriate services based on assessments.
• Act as a liaison between clients and service providers, ensuring coordinated care.
• Obtain necessary authorizations for information exchange.
• Collaborate with other service providers to plan and coordinate treatment across clients' home, school, and community environments.

Qualifications & Skills

• MHRT-C certification and a bachelor's degree.
• Minimum of one year of experience in social services.
• Must pass a background check.
• Organized, detail-oriented, and capable of working independently in the field.
