Responsibilities
- Assist clients in identifying services, programs, and resources to promote independence and achieve personal goals.
- Provide support, advocacy, referral, and assessment services tailored to individual needs.
- Understand clients' diagnoses and recommend appropriate services based on assessments.
- Act as a liaison between clients and service providers, ensuring coordinated care.
- Obtain necessary authorizations for information exchange.
- Collaborate with other service providers to plan and coordinate treatment across clients' home, school, and community environments.
Qualifications & Skills
- MHRT-C certification and a bachelor's degree.
- Minimum of one year of experience in social services.
- Must pass a background check.
- Organized, detail-oriented, and capable of working independently in the field.
