Full-Time Case Manager

About the role

This role requires strong critical thinking, effective communication, and decisive judgment to lead and build positive relationships with patients, families, and healthcare teams.

Responsibilities

• Provide education and resources to ensure appropriate care progression, utilization of services, and safe patient transitions.

Qualifications

• A minimum of one year experience in hospital-based Case Management or five years in Healthcare.
• A graduate of an accredited Nursing Program.
• Basic Life Support (BLS) certification upon hire.
