Float Patient Care Coordinator

As a Float Patient Care Coordinator, you’ll coordinate care from admission through discharge, advocate for patients and families, collaborate with treatment teams and community resources, and keep plans organized, updated, and compliant. You’ll bring empathy, organization, and problem-solving to every shift.

Responsibilities

• Care coordination from admission through discharge
• Advocacy for patients and families
• Collaboration with multidisciplinary teams and community resources
• Organizing, updating, and ensuring compliance of care plans

Requirements

• Combination of Bachelor’s degree in a human services field with at least 1 year of full-time experience serving people with mental illness
• PREFERRED: 2 years of experience in case management or patient care in a healthcare or social services delivery system
• PREFERRED: Experience in an inpatient care setting

Qualifications

• High school diploma or equivalent
• Valid driver’s license
• Ability to work flexible hours and weekends

Pay

This position starts at $23.65 per hour.
