You’ll float between our Somersworth and Portsmouth locations — supporting multiple primary care teams, learning new workflows, building cross-site relationships, and making a direct impact on patients from all backgrounds. What You’ll Do As a Primary Care Nurse Float, you will: Rotate across all primary care teams, providing nursing support exactly where it's needed mostDeliver compassionate, patient-centered care — in person and over the phoneSupport patient education, care coordination, follow-up calls, and result managementAssist with clinical needs during nurse visits and provider visitsCollaborate with MAs, providers, behavioral health, and family support teamsHelp ensure smooth patient flow and exceptional care experiences Floating between Somersworth Sections means you become a key connector across our network — gaining experience, visibility, and the chance to build relationships everywhere you go.
