Field Sales Representative- Phoenix, AZ
About the role
We are transforming care for high-risk patients with multiple chronic conditions. As a Field Sales Representative, you will engage face-to-face with Monogram-eligible patients who could benefit from our services but are not currently enrolled in our program. By educating patients on Monogram, answering their questions, and building their trust, you’ll help connect them to life-changing care.
Responsibilities
- Cultivate relationships with potential members through daily in-person outreach to high-risk, hard-to-reach patients by visiting their homes unannounced
- Educate patients in-person about the Monogram Health program
- Proactively schedule and complete at least 15 home visits daily
- Drive a designated territory daily to engage new and existing members
- Meet or exceed sales targets and Key Performance Indicators (KPIs)
- Facilitate a high completion rate of scheduled visits
- Collaborate with internal sales and marketing teams to optimize outreach strategies
- Maintain accurate records in CRM systems
Requirements
You must have previous experience as a Field Sales Rep, SDR, BDR, Account Executive, or similar role, though it is not required. Comfortable with outdoor, door-to-door, or on-location outreach. Must have a valid driver's license and reliable vehicle for travel and be willing to travel heavily daily. Strong interpersonal and communication skills. High level of self-motivation and independence. Results-driven, with a hunter mentality. Ability to adapt to different customer types and field scenarios. Tech-savvy enough to manage CRM updates, email follow-ups, and basic reporting.

Pay
The base salary plus bonus range for this role is $65,000 to $130,000 and reflects the range of experience and qualifications we may consider. Final compensation will be determined based on geographic location and job-related factors such as experience, skills, education, certifications, and internal compensation considerations. While the full range is posted, most offers will typically fall within a narrower band based on these factors. This range is inclusive of a monthly bonus, based upon individual performance. Bonus payments are not guaranteed and are awarded at the company’s discretion.
