Field Care Coordinator - Warsaw VA and surrounding Markets

About the role

The Field Care Coordinator assesses, develops, and implements personalized care plans for enrollees to promote their independence and self-sufficiency. This role involves both field visits and remote work, focusing on delivering compassionate, comprehensive care aligned with professional standards and policies.

Responsibilities

• Conduct in-home and community assessments to identify medical, behavioral, functional, and social needs.
• Collaborate with care teams and community resources to coordinate care and support transitions from acute or skilled facilities.
• Provide education and coaching to enhance member self-management and wellness.
• Advocate for members to ensure their needs and preferences are met.
• Promote proactive discharge planning and support natural support systems.

Requirements

• Licensed Practical Nurse, Social Worker, or related field with a 4-year degree.
• 3+ years of care coordination or healthcare experience.
• Proficiency with MS Office.
• Experience working with diverse populations.
• Valid driver’s license and reliable transportation for travel within the territory.

Preferred Qualifications

• CCM certification.
• Experience with Medicaid/Medicare, team-based care, or geriatric populations.
