About the role
The Field Care Coordinator assesses, develops, and implements personalized care plans for enrollees to promote their independence and self-sufficiency. This role involves both field visits and remote work, focusing on delivering compassionate, comprehensive care aligned with professional standards and policies.
Responsibilities
- Conduct in-home and community assessments to identify medical, behavioral, functional, and social needs.
- Collaborate with care teams and community resources to coordinate care and support transitions from acute or skilled facilities.
- Provide education and coaching to enhance member self-management and wellness.
- Advocate for members to ensure their needs and preferences are met.
- Promote proactive discharge planning and support natural support systems.
Requirements
- Licensed Practical Nurse, Social Worker, or related field with a 4-year degree.
- 3+ years of care coordination or healthcare experience.
- Proficiency with MS Office.
- Experience working with diverse populations.
- Valid driver’s license and reliable transportation for travel within the territory.
Preferred Qualifications
- CCM certification.
- Experience with Medicaid/Medicare, team-based care, or geriatric populations.
