About the role

The Field Care Coordinator plays a vital role in promoting member independence by conducting in-home assessments, developing personalized care plans, and coordinating services with healthcare providers and community resources.

Responsibilities

• Engaging members face-to-face or via phone to assess medical, behavioral, social, and functional needs
• Creating and implementing person-centered care plans for chronic condition management and health promotion
• Supporting care transitions, discharge planning, and advocating for members and families
• Collaborating with interdisciplinary teams to ensure comprehensive, cost-effective care
• Providing education to empower members in self-management

Qualifications and Skills

• Virginia-licensed LPN or relevant degree in Social Work/Human Services with 3+ years of care coordination or healthcare experience
• Proficiency with MS Office; experience working with diverse, potentially disabled or elderly populations
• Reliable transportation and valid driver’s license for travel within the territory
• Preferred certifications (e.g., CCM), experience with Medicaid/Medicare, team-based care, and geriatric populations
• Ability to navigate multiple locations and transport equipment
