About the role
The Field Care Coordinator assesses, plans, and advocates for enrollees to promote their self-sufficiency and independence. They conduct in-home and facility assessments, develop person-centered care plans, and collaborate with care teams, providers, and community resources to deliver comprehensive, cost-effective care.
Responsibilities
- Support care transitions for enrollees
- Educate members on self-management techniques
- Advocate for enrollee needs within the care system
- Conduct in-home and facility assessments

Requirements
- Active LPN license in Virginia or a related degree
- 3+ years of care coordination or behavioral health experience
- Proficiency with Microsoft Office
- Reliable transportation for travel within the assigned territory
Preferred Qualifications
- CCM certification
- Experience with Medicaid/Medicare populations
- Experience with team-based care models
- Experience in long-term care settings
