About the Opportunity

The Field Care Coordinator is responsible for managing care for members with low-to-medium complexity medical and behavioral needs. This role supports members' medical, behavioral, and socioeconomic needs to ensure appropriate service utilization and enhance quality of care. The position involves significant fieldwork, visiting members in homes or long-term care facilities, and requires flexibility and adaptability to effectively support diverse situations.

Responsibilities

  • Conduct comprehensive needs assessments of members’ medical, behavioral, functional, cultural, and socioeconomic conditions
  • Develop and implement person-centered care plans addressing chronic conditions, wellness, social determinants of health, medication management, and safety
  • Collaborate with internal care teams, providers, and community partners to execute care plans
  • Educate and coach members to support self-management and healthy lifestyle changes
  • Facilitate proactive discharge planning and coordinate care transitions after emergency, inpatient, or skilled nursing facility admissions
  • Advocate for members and families to ensure their needs and preferences are represented within the healthcare team

Requirements

  • Minimum 2 years of case management experience with individuals diagnosed with Serious Mental Illness (SMI)
  • Minimum 1 year of case management experience with elderly or persons with physical or developmental disabilities
  • Proficiency in MS Office applications including Word, Excel, and Outlook
  • Access to reliable transportation and ability to travel within Pinal County, AZ
  • Valid driver’s license
  • Preferred: Bachelor’s degree in Psychology, Special Education, or Counseling, CCM certification, experience in team-based care and managed care settings, social work experience, bilingual abilities

Physical Requirements

  • Ability to transition between office and field locations multiple times daily
  • Ability to navigate various terrains for member and provider visits
  • Ability to transport necessary equipment for field visits
  • Capability to remain stationary for extended computer or tablet use

Pay Range and Compensation Package

Hourly pay ranges from $24.00 to $43.00 based on full-time employment, education, experience, certifications, and local market factors. Comprehensive benefits package including incentive and recognition programs, equity stock purchase, and 401(k) contributions, subject to eligibility.
