Field Based Community Health Worker

About the Role

The Field Based Community Health Worker identifies and refers clients to social services, addressing social determinants of health such as transportation, housing, and food access. They provide home and social resource education, collaborate with healthcare teams, and develop personalized care plans to support patient health outcomes.

Responsibilities

• Assess, plan, and implement individualized care strategies
• Utilize community resources for effective case management
• Coordinate with patients, families, and healthcare providers
• Identify and initiate referrals for social support programs
• Communicate health information to stakeholders
• Advocate for patient needs and document all activities
• Engage members face-to-face or telephonically, set SMART goals, and monitor progress
• Support post-discharge activities and connect members to social services

Requirements

• High School Diploma or GED
• 2+ years of field experience
• Knowledge of community resources and culture, especially in Miami, FL
• Bilingual in Spanish and English
• Reliable transportation and ability to travel locally
• Proficiency in MS Office and strong organizational skills
