Field Based Clinical Care Coordinator - Tippecanoe County, IN

Role Overview

The Integrated Care Coordinator is responsible for managing care for older adult members with complex medical and behavioral health needs. They conduct comprehensive assessments, develop personalized, person-centered care plans, and collaborate with internal teams, providers, and community resources to implement these plans. The role involves providing referrals, supporting members' medical, behavioral, and social needs, and promoting appropriate service utilization to improve health outcomes.

Qualifications & Skills

• Master's in Social Work or a Bachelor's with relevant experience in case management for older adults or individuals with disabilities
• 2+ years in long-term support services or working with older adults
• Experience with behavioral health providers, community support organizations, and addressing Social Determinants of Health (SDoH)
• Proficiency in MS Office and ability to travel within assigned territory
• Valid driver's license and reliable transportation
