About the role
The Integrated Care Coordinator manages complex, high-risk members, primarily older adults, focusing on medical, behavioral, and social needs to promote quality care and appropriate service utilization.
Responsibilities
- Serve as the primary care manager and service coordinator for members with diverse health needs
- Conduct comprehensive assessments covering medical, behavioral, functional, cultural, and socioeconomic factors
- Develop and implement personalized, person-centered care plans with clear goals and interventions
- Collaborate with care teams, providers, and community resources to execute care plans
- Provide referrals and linkage to internal and external support services
Qualifications & Skills
- Master's in Social Work or Bachelor's in Social Work, Psychology, Counseling, or related fields with relevant case management experience
- At least 2 years of experience supporting older adults or with long-term services
- Experience working with behavioral health providers and addressing social determinants of health
- Proficiency in MS Office and reliable transportation with a valid US driver's license
- Ability to travel within the assigned territory
Preferred Skills
- Training in Motivational Interviewing, Trauma-Informed Care, or Person-Centered Care
- Experience in team-based care and serving individuals with co-occurring disorders
- Bilingual abilities are a plus
