About the role

The Integrated Care Coordinator manages complex members, primarily older adults with medical and behavioral health needs, to promote appropriate service utilization and improve care quality.

Responsibilities

• Conducting comprehensive assessments
• Developing individualized, person-centered care plans
• Collaborating with care teams, providers, and community resources
• Providing referrals to support services such as housing, transportation, and healthcare providers

Qualifications

• Master's in Social Work or a relevant bachelor's with at least 1+ years of case management experience with older adults or individuals with disabilities
• 2+ years of experience in long-term support services
• Familiarity with behavioral health providers
• Proficiency in MS Office
• Travel within the territory is expected, with access to reliable transportation
• Physical ability to visit locations and work in office and field settings

Preferred skills

• Certifications in Motivational Interviewing
• Experience with co-occurring disorders
• Bilingual abilities
