About the role
Manage complex older adult members with medical and behavioral health needs.
Responsibilities
- Serve as the primary care manager and service coordinator, conducting comprehensive assessments of medical, behavioral, functional, cultural, and socioeconomic (SDoH) needs.
- Develop and implement personalized, person-centered care plans with clear goals and interventions.
- Collaborate with care teams, providers, and community resources to execute care plans.
- Provide referrals and linkages to internal and external support services, including housing, pharmacy, and behavioral health providers.
- Engage members face-to-face and via phone to promote appropriate service utilization and improve health outcomes.
Qualifications & Skills
- Master's Degree in Social Work or Bachelor's Degree with relevant case management experience with older adults or individuals with disabilities.
- 2+ years in long-term support services or working with older adults.
- Experience with behavioral health providers and community support organizations addressing SDoH.
- Proficiency in MS Office and ability to travel within the assigned territory.
- Residence in Indiana with reliable transportation and a valid driver's license.
- PREFERRED SKILLS: Certifications in Motivational Interviewing, Trauma-Informed Care, or Person-Centered Care, Experience in team-based care and serving individuals with co-occurring disorders, Bilingual abilities are a plus.
