Field Based Clinical Care Coordinator - Shelby, Hancock County, IN

About the role

The Integrated Care Coordinator is responsible for managing complex, high-risk members, primarily older adults with medical and behavioral health needs.

Responsibilities

• Conduct comprehensive assessments covering medical, behavioral, functional, cultural, and social determinants of health (SDoH)
• Develop and implement personalized, person-centered care plans to improve health outcomes and quality of life
• Collaborate with care teams, providers, and community resources
• Provide referrals and linkage to appropriate services

Qualifications

• Master's in Social Work or relevant degree with case management experience
• At least 2 years working with older adults
• Proficiency with MS Office
• Physical ability to travel and navigate multiple locations

Skills

• Preferred certifications in Motivational Interviewing and Trauma-Informed Care
• Experience with co-occurring disorders or managed care preferred

Pay

$29 to $52 per hour, with benefits and incentives offered.

Schedule

Travel within the assigned territory is required, with remote work flexibility within Indiana.
