About The Role
A Case Planner at Rising Ground provides constructive services to children in foster care to improve their functioning. They also connect parents to services addressing family challenges and assist with reunification with their child. The primary duties include:
- Provide direct case planning services focusing on child safety, wellbeing, and permanency.
- Maintain mandated contacts with caretakers, children, parents, siblings, clinical and medical staff, schools, and other resources.
- Develop service plans and connect children and/or parents to appropriate services based on their individual needs.
- Document progress notes and complete timely, accurate reports, including FASP, PH reports, and intra-agency reporting.
- Make home visits and conduct collateral contacts to meet ACS and OCFS requirements.
- Attend Family Court hearings.
- Facilitate and actively participate in six-month Service Plan Reviews and Family Team Conferences.
- Establish a culturally appropriate network with other systems within the community.
Qualifications
- Bachelor’s Degree and knowledge of child welfare system (1 year of experience preferred).
- Good verbal and written communication skills are essential.
- Computer proficiency, knowledge of Connections, and bilingual (English/Spanish) a plus.

Work Environment
This is a full-time position located in Brooklyn or Bronx. Work hours are Monday through Friday, with options of 9:00 am - 5:00 pm, 9:30 am - 5:30 pm, or 10 am - 6 pm. Additional requirements include being authorized to work in the U.S., ability to work in-person in New York City for at least 3 days or more per week, and the ability to travel to other Rising Ground sites if required.

The general process involves reviewing the application by a Talent Acquisition Team member, followed by a phone screen, interviews with the Hiring Manager and key partners, and a final decision and feedback.
