About the Role
The Case Planner works collaboratively with therapists, education specialists, and social workers to manage a caseload of approximately 10 children with varying mental health needs. The role focuses on providing intensive casework and field services to support permanency goals.
Responsibilities
- Understand and promote the organization’s Mission, Vision, and Core Values to ensure alignment with organizational policies and procedures.
- Work with a dynamic team of professionals including therapists, education specialists, and social workers in our foster care program for children.
- Manage a caseload of approximately 10 children with varied levels of mental health needs.
- Provide intensive casework services and fieldwork services for children and their birth and foster families with the goal of providing permanency as quickly as possible.
- Conduct outreach to connect children and families with agency and community treatment resources.
- Utilize CONNECTIONS, the child welfare computer system.
- Coordinate Family Team Conferences.
- Actively participate in case consultations.
- Maintain written case records.
- Perform other duties as assigned.
Requirements
- Master’s Degree in Social Work, Psychology, or related field.
- Foster Care experience preferred.
- Bilingual (Spanish/English) preferred.
- Valid Driver’s License.
Skills
- Excellent verbal, writing, and organizational skills.
- Flexibility and ability to work under pressure and meet deadlines.
- Computer literacy.
Schedule
The incumbent is expected to work a schedule, whether in-person or hybrid, as determined by the department's needs to facilitate effective collaboration with the team.
