Case Planner (SBC)

Position Summary: The Solution Based Casework (SBC) Case Planner assists parents/caregivers in addressing barriers to their children’s health, safety, and well-being so families can stabilize, strengthen, and reduce future involvement with the child welfare system. Case Planners focus on safety-focused assessments, intervention planning, and helping families activate change. This role offers early-career social workers broad experience in a supportive environment with strong team culture and clinical supervision for those interested.

Annually serving families at over 40 sites, Children’s Aid focuses on cradle-through-college pathways, emphasizing children’s health, family stability, education, and social-emotional development.

Responsibilities

• Maintain an average caseload of 12 families with a minimum of 2 contacts per month, including 1 in-home visit.
• Visit all families and children on caseload monthly.
• Complete required documentation for the SBC model and ACS.
• Provide services including family/individual therapy, advocacy (Public Assistance, Child Support, Housing, School systems), educational monitoring, parenting support, and general case management.
• Receive training and certification in the Solution Based Casework Model and adhere to model fidelity.
• Collaborate with and refer to appropriate service providers for case coordination.
• Maintain up-to-date progress notes, Family Assessment and Service Plans, and input contact data into PROMIS and CONNX systems.
• Obtain required documents for ACS, such as school and medical records.
• Participate in weekly individual supervision, monthly group supervision, and other professional development activities.
• Schedule and participate in Family Team Conferences for all cases.
• Attend staff meetings, division events, and core training at Children’s Aid.
• Complete required training hours through the Workforce Institute (ACS).
• Perform other duties as assigned.

Requirements

• Bachelor’s degree or bachelor’s in social work required.

• Bilingual in Spanish/English preferred.

Pay

$27.47 - $30.22 per hour.
