Case Planner

Children’s Aid’s Enhanced Family Foster Care program places children with supportive families in their own neighborhoods when their parents cannot offer them the stability, safety, and support they need to thrive. The Foster Care program is prepared to deal with the widest range of child welfare situations—from temporary situations that allow birth families to address issues until they can be reunited, to specialized homes with highly trained foster parents for children who need constant care for chronic medical and developmental conditions, or children reacting to trauma. Our programs provide trainings to support individuals in these roles.

About the Organization

Children’s Aid helps children in poverty to succeed and thrive by providing comprehensive support to children and their families in targeted high-needs New York City neighborhoods. We annually serve children and their families at over 40 sites in New York, as well as through three national program models. Our efforts focus on creating cradle-through-college pathways in high-need neighborhoods, with an emphasis on children’s health, family stability, education, and social and emotional development.

Responsibilities

• Work with a dynamic team of professionals including therapists, education specialists, and social workers in our foster care program for children.
• Manage a caseload of approximately 10 children with varied levels of mental health needs.
• Provide intensive casework and fieldwork services for children and their birth and foster families with the goal of achieving permanency as quickly as possible.
• Conduct outreach to connect children and families with agency and community treatment resources.
• Utilize CONNECTIONS, the child welfare computer system.
• Coordinate Family Team Conferences.
• Actively participate in case consultations.
• Maintain written case records.
• In-person collaboration is an essential function of the job.
• Perform other duties as assigned.

Requirements

• Bachelor’s Degree required; Master’s Degree preferred in Social Work, Psychology, or a related field.
• Foster Care experience preferred.
• Bilingual (Spanish/English) preferred.
• Valid Driver’s License.

Skills

• Excellent verbal, writing, and organizational skills.
• Flexibility and ability to work under pressure and meet deadlines.
• Computer literacy.

Pay

$50,000 - $55,000/yearly
