SUMMARY
The Family Support Specialist (CHW) will support the Manager of Family Support in all activities aligned with family support, outreach, and education. Responsibilities include building on specific knowledge areas to support other team members and serving the community, particularly focusing on COVID-related activities.
Essential Job Functions
- Support families in a new Community Health Worker program by engaging in health promotion and coaching, conducting assessments, setting goals, and creating action plans.
- Navigate and coordinate health system resources, such as helping patients with follow-up care, preventive care, adherence to treatment plans, and accessing Medicaid-covered services.
- Help members find and access relevant community resources and attend appointments with them.
- Provide health education and training, develop curricula, and present trainings to families and community members.
- Participate in outreach opportunities and community meetings, and support private Facebook page activities.
- Train to lead support groups and enter pertinent information into the Electronic Record Portal.
Minimum Qualifications
- A high school diploma or GED, required.
- A parent of a child with autism spectrum disorder or a relative preferred.
- Be a Certified Community Health Worker or willing to take certification training.
- Bilingual preferred.
- A flexible work schedule, including some nights and weekends.

$18.00-$25.82
