High Point & Affiliated Organizations is a health and human service agency dedicated to treating and preventing substance use disorders and mental illness. We offer a range of programs across Southeastern Massachusetts, including inpatient, outpatient, residential, and community-based services. Qualifications
- Bachelor’s Degree level degree in Psychology or a related human services field (preferred), or equivalent experience
- 2 years of experience in behavioral health and/or inpatient psychiatric setting
- Preference for someone with lived experience as a caregiver and 1 year experience with navigating the system
- Bi-lingual/Multi-lingual applicants are strongly encouraged to apply
- Excellent interpersonal and communication skills while maintaining professional boundaries
- Maintain mental alertness and attention to high detail
- Maintain a neat and professional appearance and provide excellent customer service as needed
- Follow established security protocols while using initiative and good judgement without bias
Duties & Responsibilities
- Help to support an open access model for screening and assessments for mental health, substance use, and co-occurring treatment services within the CBHC service model
- Provide initial screenings and assist patients and families with accessing CBHC services
- Absorb and document collateral contacts with other providers such as primary care physicians, probation, parole, DCF, family members, natural supports, etc.
- Coordinate patient transportation services, as needed
- Participate in patient outreach and engagement to support access to treatment
- Support transitions of care through care coordination and discharge planning activities
- Engage in community outreach activities as needed
- Must have experience as a caregiver or parent of a youth with social-emotional needs
- Work directly with caregivers to provide support, education and guidance
- Perform other duties as assigned, including providing security support for the program to monitor patient and staff safety as part of their duties
Requirements
- Must present as courteous and professional at all times
- Good case management skills
- Experience working in the mental health/substance use field
- Demonstrated flexibility and ability to perform multiple tasks
- Excellent communication skills, including the ability to organize ideas in logical and clear fashion
- The ability to work with others in a cooperative and collaborative manner
- TB screening
- Certification in the HPTC approved de-escalation program
- Strong and effective verbal, written and organizational skills
- The ability to provide leadership and program development within an outpatient site
- Knowledge of major clinical therapeutic models and behavioral health best practice
- Strong organizational, verbal and written communication skills
