About the Role

This position is responsible for identifying children with developmental delays or special health care needs, determining eligibility for the Early Support for Infants and Toddlers Program (ESIT), and assisting families in accessing needed services and resources to achieve positive outcomes. Responsibilities

  • Assist parents of children identified with developmental delays in determining eligibility and gaining access to early intervention services.
  • Coordinate evaluations and/or assessments for referrals received to determine eligibility for early intervention services.
  • Develop, review, and periodically evaluate the Individual Family Service Plan (IFSP).
  • Facilitate input of parents and service providers in the IFSP process, including therapists, school district staff, medical providers, and other professionals involved in the care of the child to identify child/family-centered outcomes.
  • Link families to appropriate resources necessary to best meet the needs of their child and monitor the delivery of services—serve as the single point of contact in helping parents obtain needed services and assistance.
  • Coordinate the transition to school district services.
  • Provide consultation in writing and/or verbal reports to service providers involved with the care of the child within 48 hours of intervention.
  • Maintain records that document interventions provided in accordance with program requirements and guidelines.
  • Develop and maintain relationships with community agencies relevant to program functions for referral needs.
  • Effectively manage caseload by providing timely interventions, quality services, achieving positive outcomes, and maintaining productivity levels.
  • Perform home visits and travel to remote clinic sites when necessary.
  • Promote a positive work culture and uphold clinic policies and procedures, including the WE CARE standards of behavior.
  • Uphold confidentiality at all times regarding CBHA activities, reports, financials, patient health information, and other proprietary information.
  • Other duties as assigned.
  • Must have a valid Driver’s License.

Requirements

  • High school diploma or equivalent; completion of a certificate program as a Family Resources Coordinator (FRC) or the ability to obtain within one (1) year of employment.
  • Experience working directly with young children and families in early intervention, early childhood development, social services, healthcare, or a related field.
  • Specific knowledge and training in early childhood development and early intervention services.
  • Proficiency in speaking, reading, writing, and translating both English and Spanish.

Skills

  • Basic to advanced written and verbal communication skills.
  • Computational and computer skills, including:
    • Microsoft Word for basic correspondence and reports.
    • Microsoft Excel for creating tables and simple displays of information.
    • Microsoft PowerPoint for basic presentations.
    • Outlook for basic email functions.
    • Ability to enter and manage data in a centralized electronic health record system.
  • Ability to compile, analyze, and synthesize information into effective written reports, correspondence, or documentation.
  • Strong interpersonal skills, including tact and diplomacy in resolving mild conflicts or disagreements.
  • Ability to influence and convince others to take specific actions or adopt desired behaviors.
