About the role
The Advanced Practice Practitioner in Family Practice (NP or PA) plays a critical role in delivering comprehensive primary care services to diverse patient populations. This position focuses on performing thorough clinical assessments, diagnosing and managing acute and chronic conditions, and developing individualized treatment plans to promote optimal health outcomes. The practitioner collaborates closely with physicians, specialists, and other healthcare professionals to ensure coordinated and effective patient care. Additionally, the role involves educating patients and their families on health maintenance, disease prevention, and medication management while adhering to all regulatory and safety standards. Ultimately, this position aims to enhance access to quality healthcare within the community by providing advanced clinical expertise and compassionate care.
Responsibilities
- Conduct detailed patient histories and physical examinations to assess health status and identify medical conditions.
- Order, interpret, and follow up on diagnostic tests and laboratory results to inform clinical decision-making.
- Develop and implement evidence-based treatment plans, including prescribing medications in compliance with DEA regulations.
- Provide patient education on disease prevention, lifestyle modifications, and medication adherence to support long-term wellness.
- Collaborate with interdisciplinary healthcare teams to coordinate patient care and facilitate referrals to specialists as needed.
- Maintain accurate and timely documentation of patient encounters in electronic health records.
- Respond promptly to patient inquiries and manage urgent clinical situations using advanced clinical skills and Basic Life Support techniques.
Skills
- Required: Clinical assessment and advanced clinical practice, proficiency in DEA regulations, BLS certification, experience with electronic health records, and additional training in chronic disease management or geriatric care.
- Preferred: Experience with telehealth platforms, previous work experience in a multidisciplinary healthcare environment, participation in continuing education programs related to family practice or primary care, and two years of primary care experience.

Pay
$110,000 - $150,000 per year
