Family Practice/Primary Care Nurse Practitioner

About the role

We are seeking a dedicated and compassionate Nurse Practitioner to join our Primary Care team. In this role, you will provide comprehensive, patient-centered care to individuals across the lifespan, focusing on health promotion, disease prevention, and management of chronic conditions.

Responsibilities

• Provide primary healthcare services to patients in a primary care setting
• Conduct physical exams, order and interpret diagnostic tests, and develop treatment plans
• Manage chronic conditions and coordinate care with specialists as needed
• Offer preventive care services such as immunizations and screenings
• Collaborate with other healthcare professionals to ensure holistic patient care

Requirements

• Current Arizona RN license
• Master's degree in Nursing (MSN) from an accredited program
• ACNP certification preferred
• Experience in primary care settings preferred
