Responsibilities

• Provide comprehensive primary care across the lifespan.
• Diagnose and manage acute and chronic medical conditions.
• Perform preventive health exams and wellness visits.
• Order, interpret, and manage diagnostic testing.
• Prescribe medications and treatment plans within New Hampshire scope of practice.
• Collaborate with an integrated multidisciplinary healthcare team.
• Document patient care accurately within the electronic health record.
• Participate in quality improvement initiatives and provider collaboration activities.

Requirements

• Current New Hampshire RN and APRN Licensure
• National APRN Board Certification
• Current DEA Registration (or ability to obtain before employment)
• Current American Heart Association (AHA) Basic Life Support (BLS) Certification
• Minimum of three (3) years of licensed practice as an Advanced Practice Registered Nurse in Primary Care, Family Medicine, Internal Medicine, Community Health, or a comparable outpatient setting
• Family Nurse Practitioner (FNP) certification is strongly preferred. Other APRN specialties with substantial primary care experience are encouraged to apply.
• Federally Qualified Health Center (FQHC) experience
• Community Health experience
• Population health initiatives
• Team-based outpatient care
• Electronic Health Record (EHR) proficiency
