Family Nurse Practitioner Position in Phoenix; Integrated Care; 44th St/ Oak

Responsibilities

• Conducts physicals/check-ups, medical evaluations, diagnostics, treats illnesses, orders lab tests, provides general and preventative care, prescribes and monitors medications, coordinates with other providers, makes specialty care referrals, and educates patients about preventative care.
• Establishes therapeutic relationships with a variety of patients for the purpose of establishing and reaching appropriate treatment goals, within the context of an integrated setting.
• Performs independently or as a part of a treatment team.
• Provides leadership and cohesion within the clinic environment, and may provide direct supervision to other professional and/or non-professional medical staff, and students.
• Participates in program evaluation procedures and professional record keeping, makes referrals appropriately for specialty and behavioral health care.
• Provides direct patient services not less than 85% of the work time.
• Employs therapeutic interventions appropriate for identified treatment issues.
• Seeks appropriate clinical supervision/support, when necessary, to ensure the provision of effective treatment.
• Establishes appropriate patient files that conform to agency guidelines, best professional practice and State licensing regulations.

Requirements

• Full-time position (4-10's schedule) based in Phoenix, AZ near 44th St/Oak.
• Certification Required: AZ FNP License Required.
• FNP Experience Preferred.
• NextGen Experience is a PLUS.

Pay

• MSN: $115K–$120K+ DOE.
• DNP: $129K+.
• Additional Language Differential Pay Available for Spanish, Vietnamese, Korean, Arabic, Farsi, Swahili or ASL: $3/hour ($6,240 annually).
