Family Nurse Practitioner (FNP) | PRN Health Risk Assessments | Mercer County, NJ

Responsibilities

• Covers comprehensive in-home health risk assessments to identify active and chronic disease conditions, as well as physical, mental, and social needs.
• Takes patient history, performs examinations, determines diagnoses, and provides written documentation according to NCQA standards.
• Takes patient vital signs, places case management referrals, and communicates with PCPs as necessary.
• Communicates with patients, caregivers, agency nurses, other providers, and vendors to ensure proper diagnosis.
• Maintains patient confidentiality and observes OSHA Universal Precautions.
• Attends required meetings and in-services, participates in committees, and engages in professional development activities.
• Works collaboratively with teammates, physicians, medical staff, patients, caregivers, agency nurses, providers, and vendors.

Qualifications

• Active/unrestricted nurse practitioner license to practice in coverage states.
• Board certification in one of the following: ANCC, AANP, or NCCPA.
• Active BLS Certification.
• Current enrollment in Medicare/Medicaid.
• Maintain a valid driver’s license and good driving record.
• Outstanding EHR skills.
• Geriatric training/experience preferred.
• Teamwork and effective working relationships with patients, medical staff, and the public.

Location Requirement

To be considered for this position, candidates must reside within Mercer County, NJ. This requirement is based on the density of our patient population in the area.
