UK Healthcare is looking for a Family Navigator to serve as a vital connector between patients, caregivers, and the care team, ensuring individuals feel supported, informed, and empowered throughout their healthcare journey. This role blends compassionate, patient-centered support with program development and operational excellence—building and strengthening navigation resources, driving collaboration across teams, and advancing initiatives that improve access, adherence, and overall experience.

About the role

The Family Navigator plays a key role in removing barriers to care, enhancing communication across the continuum, and using data and partnerships to continuously elevate the impact of the family navigation program.

  • Guide and support patients and caregivers

  • Drive program impact and accountability

Responsibilities

  • Serve as a connector between patients, caregivers, and the care team
  • Ensure patients and caregivers feel supported, informed, and empowered throughout their healthcare journey
  • Remove barriers to care and enhance communication across the care continuum
  • Develop and strengthen navigation resources and programs
  • Drive collaboration across teams to improve access, adherence, and overall experience
  • Use data and partnerships to advance initiatives and elevate program impact

Requirements

  • Bachelor’s degree (or equivalent experience)
  • 5 years of related experience
  • Obtain approved national certification in area of specialty within 24 months of hire
  • Knowledge of and experience working with community-based resources providing services to the identified patient population
  • Proficiency with electronic medical records
  • Strong communication and problem-solving skills

Preferred Qualifications

  • Bachelor’s degree plus at least 5 years of experience working with children/families who have developmental and behavioral diagnoses, including autism, pervasive developmental disorders, learning disabilities, and cognitive impairments

  • Sitting and standing for extended periods of time

Pay

$43,680–71,323 per year

Schedule

Primarily Monday–Friday, 8:00 a.m. to 5:00 p.m.
